Online birth certificate (if applicable) or photocopy of certificate of passing SSC or equivalent examination or any other public examination along with the completed Form-2 of the person concerned for registration; Registration has to be completed by submitting copy of Citizenship Certificate, Certificate / House Rent / Holding Tax / Receipt of payment of any utility bill.
